I fully understand and assume all associated risks. I willingly consent to engage in the programs conducted by Lorenzo D. Rivers, a Certified Strength and Conditioning Specialist with Distinction and a Registered Strength and Conditioning Coach Emeritus with two decades of direct practice in the field through the NSCA, under L.D. RiversFitness, at Anytime Fitness Ruckersville, VA. This participation encompasses 1-on-1 Training, Partner Training, Group Training, Classes, as well as Online Training Plans, Zoom Sessions, and FaceTime Sessions. I also agree to disclose any physical limitations, disabilities, ailments, or impairments that might affect my participation in these programs.”
